Your social prescribing link worker can connect you with community activities to improve your health and wellbeing.

Our social prescribers Mary and Pete know that taking care of your health involves more than just medicine. They can spend time with you exploring what matters to you and help you identify activities and/or local support which could improve your health and wellbeing.  

They are available to talk about the issues you’re facing and find the right services to support you.  

Simply talk to your GP, health professional or surgery staff for more information and ask to be referred to our social prescribers Mary and Pete and they will  be in touch, it’s as simple as that!
