How to Register
Our catchment area covers the districts of Broomhill, Fulwood, Ranmoor, Lodge Moor, Crookes, Nether Green and Endcliffe.
If you are unsure whether you live within our catchment area, please ask our reception staff who will be able to help you.
In order to register you will need to bring your medical card along with proof of ID to reception to confirm your address.
If you have misplaced your medical card you will be asked to fill out a Registration form
We also request that you complete a New Patient Questionnaire which is available to download by following the links above, or collected from the surgery reception
